Thank you for your interest in the Indiana Psychological Association (IPA) Mentoring Program. The purpose of this questionnaire is twofold. First, it serves as an application for mentees and mentors and will assist us in matching you to your mentor/mentee. Second, it will allow us to gather information on how to best structure the program so we can meet your needs to the best of our ability.
